The Mechanism of Tumor Necrosis Factor(TNF) to Lung Injury and Hemodynamic Change in the Dogs


Abstract
A recent report showed that "Tumor Necrosis Factor(TNF)" administration to the dogs caused the lung injury at 24 hours after injection. Other clinical studies also reported that the TNF like activity was observed in the patients with septic
shock.
In
addition, it has been said that pretreatment with anti-TNF antibody could prevent the development of the lung injury. Therefore it might be a reasonable speculation that TNF could induce septic shock and lung injury in man and dogs. But the
mechanism of
the lung injury induced by TNF remains unclear.
According author examined the changes in chemical mediators in attempt to make clear the mechanism of the lung injury after TNF administration.
After administration of TNF, eman arterial blood pressure remained at baseline values in spite of decrease in cardiac index. Pulmonary vascular resistance and systemic vascular resistance were increased significantly from 120 minutes after TNF
administration. After administration of TNF, PaO2 was decreased slightly, but these changes were not showed statistically significant. Extravascular lung was increased at 2 hours after injection of TNF and sustained through all experimental
periods.
Endothelin-1 and Endothelin-3 were increased dramatically at 60 minutes and 120 minutes after administration of TNF, respectively. In the same manners, serum concentration of 6-keto PGF1 alpha was elevated at 60 minutes and 120 minutes and it
might
be
associated with increase in PG12 concentration after TNF administration.
@ES From above results, author can get some conclusion as follows:
@EN 1. TNF administration in low dose doesn't affect arterial blood pressure and arterial oxygenation.
2. TNF administration results in the increase of the PGI2 and PGI2 may change the vascular permeability and produce the lung injury.
3. TNF administration elevates the serum concentration of endothelin-1 and endothelin-3 which are related to vascular permeability.
Therefore author can speculate that TNF plays some roles to initiate lung injury during early sepsis and ARDS.
